To get this get you must go to Western Wastes and loot the heart from A Banished Efreeti, who is on a 1 hour respawn timer. Then you turn that heart into a drake (name escapes me atm) who is located in PoT in the little building where the zone to Plane of Fire is. Having a tracker and being on good CoV faction helps with killing the efreeti, so the dragons and drakes don't aggro you when killing him.
